Digital Health and Wellbeing service for Falmouth and Penryn. A social prescribing service for anyone who is in need of support for a range of issues, but must include a digital need. This would include people who have no access to the internet or to a device, those who would like to upskill and those who would like to increase their confidence. A blended range of 1-1 support including telephone, video calls and home visits (when safe to do so), over a period of 16 weeks; offering a safe space in which to share, to be listened to and understood and supported to make positive change. The digital health and wellbeing coach will offer advice and guidance on services and support available in the community to increase digital connectivity and confidence alongside resources to support general health and wellbeing. This might include resources to support physical and mental health, debt and money advice, work, lifestyle change and to increase social inclusion.
